Emergency Pick-up Form
Anytime your child is going home with someone not listed in your emergency contacts in FACTS, please fill out the Emergency Pick-Up Authorization Form attached and submit it to the classroom teacher and Linda Alegria, Office Manager, no later than 2PM on the day of pick-up. This form is for a one-time pick up only. You may also go into your Family Portal in FACTS to add any individual’s permission to pick up your child at all times.
